Dr. John B. o'neal, III LIFETIME ACHIEVEMENT award

The purpose of the Dr. John B. O'Neal, III Lifetime Achievement Award is to recognize and reward an individual in Georgia for his/her contributions to pre-hospital care that have been consistent and long lasting, representing in effect, a lifetime of outstanding service to the profession and to the public.

CRITERIA FOR CONSIDERATION

In your nomination statement, describe in narrative form why you feel that the nominee should receive the Dr. John B. O'Neal, III Lifetime Achievement Award.  Please include the following:

• Highlights and details from the nominee’s career in Emergency Medical Services in the State of Georgia.
• Nominee’s role in the continued enhancement of Emergency Medical Services in the State of Georgia throughout their career.
• Nominee’s involvement in activities to strengthen public awareness and support of Emergency Medical Services.

DOCUMENTATION

Submission of supporting documentation is encouraged. Examples would include newspaper clippings, commendations, award certificates, leadership positions in organizations, community involvement, public education, continuing education, etc.
