OBJECTIVE
The purpose of the Dr. Joseph E. Simon Pediatric Award of Excellence is to provide recognition and appreciation to any prehospital provider within Georgia’s Emergency Medical Services system for reducing the morbidity or mortality of a pediatric patient in the prehospital setting.
GOAL
To ensure that those who work in the prehospital environment are aware of the effects that their knowledge and skills have in the overall positive outcome of Georgia’s pediatric citizens.
ELIGIBILITY
Consideration for this award is open to any Georgia First Responder, Emergency Medical Technician – Basic, Emergency Medical Technician – Intermediate, Cardiac Technician, and/or Paramedic who submits documentation on a pediatric patient treated in the prehospital setting.
CANDIDATES AND NOMINATIONS
Any prehospital care provider, as defined above, may be nominated by anyone for this recognition by completing and submitting the attached nomination form to the Georgia EMS-C Advisory Council Awards Committee.
SELECTION
All nominations submitted will be reviewed by the EMS-C award selection committee.
Information needed on nomination form:
- Date of Call
- DHR PCR Number
- Type of Call
- Times - dispatched, on scene, enroute to ED, arrived Ed, total on scene time
- Chief complaint/mechanism of injury on dispatch
- Brief description of Initial Assessment upon arrival at patient
- Description of the complete prehospital management of the patient
- Additional Documentation that can be submitted: Emergency Department Physician’s review and/or comments; Emergency Medical Services Medical Director’s review and/or comments; additional documentation you would like the committee to review in the selection process (i.e. newspaper articles on the incident, letters from patient’s family, etc.)
